Whether with or without surgery: a sex change (性転換 which is the term in Japanese) can only ever be incomplete and in the form of an adaptation to the desired gender.
A decisive step towards adapting to transidentity and also to ensure the diagnosis is treatment with hormones. This can start before puberty. Hormones regulate the function of the reproductive organs. However, they also influence a person’s mental state and appearance. The decisive hormone for the opposite sex is possible because all people have corresponding hormone receptors.
For example, changes under the influence of appropriate hormones
It can also reduce fertility: in organic women, the regular cycle comes to a standstill. Biological men lose erection and ejaculation ability.
Not all hormonally generated changes can be reversed after stopping hormone therapy.
In practice, doctors try to influence the required testosterone or estrogen level in the blood as desired. Only after an operative sex change is a lifelong intake of hormones necessary to keep the patient healthy. Their long-term use is associated with health risks.
Gender Adaptation Woman-To-Man
The pace of physical change in female-to-male gender adjustment is determined by the number of androgen receptors available, not by the dose of testosterone administered. Depot injections with duration of action of two to three weeks are usually administered, optionally the oral administration of a testosterone preparation. The dose should be adjusted so that the patient’s liver is not stressed.
Persistent acne, for example, can occur as an unpleasant side effect. This can be treated with agents such as a broad-spectrum antibiotic. Accumulation of water has also been observed. The ovaries stop working, the menstrual period stops. A broken voice occurs only over longer periods, due to an increase (hypertrophy) of the vocal cords. The body hair increases very soon after the start of the hormone treatment, beard growth sets in. The clitoris is also growing, but only rarely to the desired extent.
